One form per child
Every child taking part needs this completed and signed by a parent or guardian before their first session. It tells us how to reach you, what we need to know about your child's health, and who is allowed to collect them.
This is a consent and medical information form, not a disclaimer of liability. Under UK and Northern Ireland law a parent cannot sign away a child's right to claim for injury caused by negligence, and nothing here attempts to do that. What you are agreeing to is that your child may take part, that you understand hurling carries a risk of injury even when run properly, and that we may act in your child's interests in an emergency.
Please give someone other than yourself, who we can reach if you cannot be contacted.
We will not release a child to anyone not named here without speaking to you first.

If you have any concern about your child's welfare, or about the conduct of anyone
involved in the camp, please raise it with the Children's Officer.
Shay is also the lead organiser of the camp. So if your concern is about him, about another organiser, or about anything you would rather not raise with us directly, you should go outside the programme — to the Ulster GAA Safeguarding team, to your Health and Social Care Trust Gateway Team, or to the PSNI. You do not need our permission and you do not have to tell us first.
The online version asks for exactly the same information as this page and takes about five minutes. It opens in a new tab. One form per child, please.
It asks you to confirm you have read the privacy notice — that is section 8 below, worth a quick look first.
Fill in the sections above, then confirm below. A signature is not needed if you are submitting online — your name, the consents you have ticked, and the date and time are recorded instead.
Submitting opens your email app with everything filled in, addressed to the camp organiser — you just press send. If nothing opens, use Copy my answers and paste them into an email to shay.deegan@gmail.com.
I confirm that I am the parent or legal guardian of the child named in section 1, that the information I have given is accurate and complete to the best of my knowledge, and that I have ticked the consents in section 6 that I agree to. I will tell the organisers if any of these details change during the programme.
The Social Slashers (registered NI charity NIC109510), as the organisers of the camp, together with Saul GAC and Carryduff Hurling as partner clubs.
Only the coaches and organisers who need it to run the camp safely. Medical details are shared with the coach leading your child's group and the first aider on duty. We will share information with emergency services if your child needs urgent treatment. We do not sell it, and we do not pass it to anyone else without your permission, unless we are required to by law or we have a safeguarding duty to do so.
Forms are destroyed three months after the final session, except where an injury or incident was recorded, in which case that record is retained as long as our insurers and the GAA require.
You can ask to see what we hold about your child, have it corrected, or have it deleted, and you can withdraw any consent at any time. Contact shay.deegan@gmail.com. If you are unhappy with how we have handled your information you can complain to the Information Commissioner's Office at ico.org.uk.
